Test kits and questionnaires

Questionnaires and test kits are tools used by speech pathologists to assess various aspects of communication and swallowing disorders in individuals, enabling them to identify areas of strengths and weaknesses, make accurate diagnoses, and develop individualised treatment plans tailored to the needs of each client.

While some questionnaires are readily accessible online, others are restricted to qualified clinicians, ensuring compliance with copyright, ethical standards, and adherence to professional guidelines.

Online questionnaires and test resources

Here is a selection of questionnaires which are available online:

Sydney Swallow Questionnaire
This is a 17 question, self-report inventory, developed to measure symptomatic severity of oral-pharyngeal dysphagia.
Neuromuscular disease swallowing status scale (NdSSS)
This 8-stage Neuromuscular Disease Swallowing Status Scale (NdSSS) was developed and tested to evaluate dysphagia.
Screening Tool of Feeding Problems Applied to Children (STEP-CHILD)
A parent-reported questionnaire using a 3-pt Likert scale to assess feeding problems like chewing issues, rapid eating, food refusal, selectivity, vomiting, and stealing food.
Child and Oral Motor Proficiency Scale (ChOMPS)
A tool used to assess the oral motor skills and proficiency of children in various activities related to eating, drinking, and speech.
The Pragmatics Profile of Everyday Communication Skills in Children
This tool was designed to provide practitioners with a comprehensive understanding of children's communication abilities in daily-life situations.
Trans Woman Voice Questionnaire (TWVQ)
This tool was developed by La Trobe's Communication Clinic to assess and address the voice-related needs of transgender women. Links to the questionnaire are available in the 'Voice gender affirming' section.
Brisbane Evidence-Based Language Test (Brisbane EBLT)
This is a comprehensive tool to assess language skills across a wide range of severity levels, including perception, auditory comprehension, verbal expression, reading, and writing.
Aphasia Rapid Test
This bedside assessment tool was designed to evaluate the severity of aphasia in acute stroke patients in less than 3 minutes.
Social Activities Checklist (SOCACT) and COMACT (Communication Activities checklist)
These self-assessment tools evaluate the communication involvement and social participation of individuals experiencing aphasia.
